Disability Travel Card Application form available at: www.easterseals.ca Click on national programs, then scroll down to the bottom of the page and click on application form. A person with a disability travelling with Via Rail or Greyhound with an attendant can take advantage of waived or discounted fare for their attendant. Alternatively, a person could contact us for a copy of these forms.
